To start this off, Sith kill off their masters to take up the role of Dark Lord of the Sith. Why? First rule of the Sith: "The strongest must rule if we are to survive." - Darth Malak - KOTOR. So the apprentice killing his/her master is to show that he/she is stronger than his/her master. This is usually done in mortal combat.
However, we know in KOTOR that Darth Malak attempted to kill Darth Revan by firing on his ship. Does that seem proving? Gahmah doesn't think so. Gahmah means, seriously, "fire on his ship". That's more of an assassination attempt than mortal combat. To other Sith, this may seem like a coward's trick. In the end, Revan was still alive, and killed Malak in mortal combat, even when he could draw upon the power of the captive Jedi. Malak wasn't stronger than Revan after all. Note that Revan was a servant of the light at the time of Malak's death.
Malak's example, however, wouldn't be the last time that happens.
According to Palpatine's story about Darth Plagueis the Wise (speaking of him, have you seen
Gahmah's design for him?), it was said that after he taught his apprentice everything he knew, the apprentice killed him in his sleep.
For all we know, Plagueis was Darth Sidious' master.
My question now is this: Was Sidious really stronger than Plagueis? According to the story, he killed him in his sleep. Like Malak almost did with Revan, this was assassination, not mortal combat. Again, to most Sith, this would seem like a coward's trick. We don't really know, since, unlike Revan, Plagueis didn't survive. It is most likely that Sidious killed him personally to make sure he was dead. It seems that Sidious learned something from Malak's mistake.
We may not know until the upcoming novel, or maybe never.
